Patients With Insurance
We accept all major health insurance coverage, including Medicaid, Medicare, and Hawk-I.

If you have insurance, please bring your insurance card to all appointments.

It is your responsibility to understand your insurance benefits. For more information about the Medicaid MCO plans, Click Here.

Patients With No Insurance
If you do not have insurance, that’s OK! We believe quality health care should be affordable to all. You will never be denied services because of your inability to pay. We offer financial assistance, including a sliding fee scale and payment plans.

Financial Assistance

You must meet minimum gross income requirements set by the Federal Poverty Guidelines to qualify for and receive financial assistance. Eligibility indicators are family income and family size. A family is defined as one or more individuals who are financially dependent as determined by the IRS.

We require verification of income for financial assistance. Refusal of an applicant to provide requested information will result in denial of financial assistance.

Financial assistance will not be granted if fraudulent information is given at the time of registration or on the Declaration of Income Form.

All third-party payments will be collected prior to providing financial assistance.

Financial Assistance applies to most medical and dental procedures. It does not apply to dental lab costs, court ordered services, or immigration documentation charges.

Sliding Fee Scale & Payment Plans

The sliding fee scale can help make your care more affordable. This scale is based on family size and income.

You will need to complete and return an Annual Financial Assistance Application so we can determine if you are eligible for sliding fee discounts or a payment plan.

Insurance Enrollment

You may also apply for Medicaid/Title 19 coverage. Patients under 18, ineligible for Medicaid/Title 19, are encouraged to apply for Hawk-I coverage. To apply online for Medicaid, use your Internet browser and go to the DHS Services Portal.

If you need help enrolling in Medicaid or an Iowa Marketplace Choice Plan, we have a certified Outreach and Enrollment Coordinator to help you.

Patient Fees

Please plan to pay any co-pays or appointment fees at the time of your visit. Cash, checks, credit and debit cards are accepted.

A statement of your account will be sent to you each month. A Benefits Counselor is available to answer questions and help determine financial eligibility for programs to assist with your health care costs.

Patient accounts or your payment plan should be up-to-date before your next appointment.
